There are times when parents want to fully change the name of the baby, or the parent’s name. In this case, you must visit the court and obtain an order. Once the order has been received, the court will discard the original birth certificate and replace it with a new one.

At times the parents decide to get married after the child is born. There are certain steps which you need to take in this case since the child’s father name has to be added if the father has not signed a declaration of paternity.

In this case the father has to submit two different forms. The first form is related to the declaration of paternity where he admits that he is the father of the newborn child. Once that form has been filed up, he then has to file up another form since the birth certificate does not mention the name of the father. The second form helps the parents add the father name to the certificate. In case there is a spelling mistake in the father’s name, the second form needs to get resubmitted.
