You must at least one identification document in order to apply for a replacement social security card. It can be a U.S. driver's license, a U.S. non-driver identification card or a U.S. passport. If you do not have any of these documents, go to the official website of the Social Security Administration and look what other documents you can use to establish your identity as a US citizen. Bear in mind that whatever identification document you use must be original because photocopies are not acceptable when applying for a replacement social security card.
Once you know that which identification document you will be using, go to the official website of the Social Security Administration once. Look for a SS-5 form and download it. This form is basically an application for a new card. Print the SS-5 form and read it thoroughly. Read instructions on how to fill the form in order to make sure that you do not make any errors while filling the form, otherwise you will end up slowing the process that the Social Security Administration need to follow in order to issue a new social security card.
Attach the required documents to the completed SS-5 form. Visit a local social security card centre along with the completed SS-5 form and submit the form to a SSA officer. If for some reason you cannot go to a local social security card centre in person, you have the option to mail the documents via regular post mail. This will take longer because SSA can start processing your application only after they receive the mail.
Once the new social security card has been issued, SSA will send it to your mailing address along with the identification documents that you submitted along with the application form. You will also receive a receipt along with the new social security card and your original identification document.
