
If you have been working somewhere for a long time, then skipping the office for any given reason may not be something new for you. Picking up the pen and paper, you simply address your boss or supervisor, mention the reason for your leave, place it on the desk and stroll whistling out of the office. Taking such a leave is easy since you have clearly mentioned the reason. However, if it is about having a fight with your wife, then there is no way in the world you are going to mention this in the leave application. Hence, all such matters are personal and the leaves taken are called personal reason leaves.

Write you name at the top followed by your designation in the next line, your address in the third and your contact number in the last line.
Leave one line spacing after mentioning your name and contact details and write the date on which you are sending this leave application.
Again leaving one line space, write the name of your supervisor with his designation followed by the name of your company and the address in the following two lines.
Leaving one line space after the above step, write down the subject and remember that it has to be bold and underlined and should just hint the reason for your leave.
Address your boss or supervisor writing Sir or Madam with the last name, otherwise simply write To Whom It May Concern.
Begin the main body of your letter by straight away putting up the request for your leave without going for a prolonged introduction.
Mention the dates of your leave or the duration and since it is a leave for a personal reason so there is no need to write anything else except Personal Reason.
Do not forget to write your date of return along with your contact number during leave.
Conclude the letter by writing a couple of sentences that would show some appreciation for your boss and the company.
