There are various types in a leave letter depending upon the number of days you intend to take off. Whether you wish to take more days off or less make sure that you have stated the reason along with the number of Leave days. Request leave letters have to be written in a formal manner by using humble and respectful words and sentences, because the leave letter will be transferred all the way to the Human Resource department. Make sure that complete address information is stated in the letter so that the right person receives your letter.

Dear Adams
I have an appointment with a dentist on the 5th of December at 12:00 pm. I tried to get the appointment after the office timings but the dentist was not available at that time. I request you to grant me short leave, I will complete my pending work the next day.
Hope to receive a positive response from your side. Thank you for your consideration.
Yours sincerely,

Respected Ma’am
I have to make a visit at my grandmother’s house in Alabama on the 6th of June 2011 due to her illness. I will have to stay there for a week because she is hospitalized and her condition is very critical. Please grant me leave for seven days starting from 6th of June 2011 till 13th of June 2011.
I will make sure to hand over my responsibilities to James Stewart. I will remain in contact with him in order to make sure the work is being done perfectly.
Thank you for your consideration.
Yours sincerely,
Laura Davis
